Initial Consultation

Your initial appointment will consist of either endodontic treatment or a consultation explaining your diagnosis and treatment options. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your endodontic referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary insurance cards and ID numbers. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation and/or treatment visit. Additionally, children under the age of 15 must have a consultation visit to determine if successful treatment is possible.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, steroids, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. Additional digital films are required for our database, and they will be taken at our facility. For additional information, please refer to the FAQ section on x-rays.

Oral Sedation

For apprehensive patients and/or those undergoing a difficult treatment procedure, we do offer oral sedation for an additional fee. This involves a sedative medication that is taken by mouth. You remain conscious and can breathe and communicate on your own, but the medication does cause drowsiness and you must be accompanied by a driver (you will be unable to drive yourself home). Patients undergoing oral sedation need to be at our office one hour prior to their scheduled treatment appointment in order to complete all paperwork prior to being sedated. We must be notified at least 24 hours prior to your appointment if you are interested in oral sedation for treatment. A pre-operative consultation and physical examination is usually necessary for patients undergoing oral sedation for treatment. Please have nothing to eat or drink 4 hours prior to your treatment. Once again, you will also need an adult to drive you home.
